Claims Reporting During the 2022-2023 Winter Holiday Closure

Insurance and Risk Management (IRM) will be closed for the Winter Closure period Saturday, December 17, 2022 through Monday, January 2, 2023. To report claims during this period and to access other IRM services:

For claims reporting, please use the third-party claims administrator’s telephonic claims centers.

Workers’ Compensation (employee injury) claims, please call 877-6UC-RPRT (877-682-7778). The Claim Form (DWC 1) (PDF) and Incident Report Form (PDF) must be filled out and scanned to

  • From Monday, 12/19/22 to Wednesday, 12/21/22: Injured employees should be referred or taken to UCLA Occupational Health Facility (OHF) office.
  • From Thursday, 12/22/22 to Monday, 01/2/23: Injured employees should be referred or taken to the Emergency Medicine Center (EMC) at Ronald Reagan/UCLA Medical Center because the OHF will be closed. Injured employees must identify themselves as UCLA employees because they are covered by the Workers’ Compensation program. Follow-up medical treatment should be scheduled with OHF when they re-open on January 3, 2023.

 Auto, Property and General Liability (injury or property damage to 3rd parties) claims, please call 800-416-4029.

  • In light of increased volume with calendar-year end, all requests to either reduce or waive insurance requirements must be received by Friday, December 2nd.  To guarantee an initial review by winter closure, all other contract review and certificate of insurance requests must be received by Friday, December 9th.

Employee Disability Management Services will be closed and information can be obtained on the Disability Management website.

  • Inquiries regarding job accommodations, including COVID-19 related accommodations, should be sent to and a response will be returned on/after January 3, 2023.

Driver Safety Training:

  • Please note that driver safety training must be requested by Tuesday, December 13th to guarantee scheduling prior to calendar-year end.
